Case 86 Discussion

Multicentric osteosarcoma.(40.322)

Involement of more than one skeletal site by osteosarcoma is known as multicentric osterosarcoma. The possibilities are as follows:

1. Osteosarcomatosis: simultaneous lesions in different sites. This is a very rare entity that is seen in children and usually consists of multiple sclerotic lesions in tubular bones.

2. Multiple, metachronous osteosarcomas: one or more new tumors develop after initial treatment of a primary osteosarcoma. This entity is usually seen in adolescents or young adults and may represent late metastases from the primary tumor.

3. Unicentric lesion with skeletal metastases.

It is often difficult to distinguish these subsets.
